After a very long process of frustration with lack of resources and incompetant doctors and therapists, I found someone willing to work with me. She met my needs, didn't feel that my age or lack of finances should preclude me from medical transition, and most importantly helped me to get a letter for testosterone. Here I'm in the parking lot of her office building waiting for her to do a drive-by letter drop off. I have been waiting for this day for years.
